Nurse Office Forms for School Year 2010-2011
| Asthma Form | This form needs to be completed and returned to the school office for all USD #218 students who are diagnosed with asthma and will use inhalers during school hours. |
| Over the Counter Medication Form | Any USD #218 student who will need over the counter medication administered during the school day, will need to complete this form and return to the office along with the medication in the original bottle. |
| Prescription Medication Form | Any USD #218 student who will need prescription medication administered during the school day, will need to complete this form and return to the office along with the medication in the pharmacy labeled bottle. |
